Medicine is the IB pathway where subject selection leaves the least room for error. The requirements are highly specific, the competition is intense, and a wrong choice at subject selection can close doors that are very hard to reopen.

The Subjects You Need

Biology HL: Non-Negotiable

Almost every medical school in the world, whether in India, the UK, Europe, or North America, requires Biology at Higher Level for IB applicants. IB Biology HL covers cell biology, genetics, the immune system, human physiology, and more. These are the foundations that a medical degree builds on.

Chemistry HL: Equally Non-Negotiable

Chemistry is the second pillar of medical preparation. Biochemistry, pharmacology, and physiology all draw heavily on a foundation in chemistry. Virtually all medical schools require Chemistry HL. For Indian students, Chemistry HL also provides a stronger foundation for NEET preparation than SL.

Mathematics: Which Level?

Medicine does not require the same mathematical depth as engineering. For most medical school applications, Mathematics AA SL is the right choice. It keeps options open across the widest range of programmes and signals solid quantitative ability. If you are considering research-intensive medicine or fields adjacent to medicine, AA HL is worth considering. For most medicine applicants, AA SL is the honest recommendation.

The Third Higher Level

With Biology HL and Chemistry HL fixed, your third HL is where you have genuine choices.

• Physics HL is the strongest choice if you want to maximise optionality. It satisfies NEET eligibility (which requires Physics HL), keeps engineering and natural science pathways open as backup, and signals the highest level of scientific preparation. For students keeping NEET as an option, Physics HL at the third HL is effectively non-negotiable.
• Psychology HL is a defensible choice for students not targeting NEET. Medical school interviews increasingly test communication skills, empathy, and ethical reasoning. A student who has spent two years studying Psychology at HL has engaged seriously with the science of human behaviour, which is directly relevant to clinical practice.
• A humanities subject at HL such as History can work for UK medical schools specifically. Oxford and Cambridge welcome applicants with diverse academic backgrounds.

Requirements by Region

• India NEET: Physics, Chemistry, and Biology at HL are required. This effectively means the triple science combination of Biology HL, Chemistry HL, and Physics HL is the only IB combination that satisfies NEET eligibility while keeping all Indian medical pathways open.
• UK medical schools: Biology and Chemistry at HL are required. Most require the UCAT or BMAT admissions test. Work experience in healthcare settings is a significant component of UK applications.
• Netherlands and Europe: English-medium undergraduate medical degrees are popular among Indian families. Biology HL and Chemistry HL are standard. Some programmes require Physics HL as well.
• US and Canada: Medical education is postgraduate in North America. IB students apply to undergraduate programmes with the intention of applying to medical school after a four-year degree.

Work Experience and CAS

For medicine, what you do outside the classroom matters more than in almost any other pathway. UK medical schools are explicit: they want evidence that applicants have spent time in healthcare settings. Biology HL and Chemistry HL are fixed. Physics HL is the strongest third science for students keeping NEET open. Mathematics AA SL is right for most medicine applicants.
